Fees are due at the time of service. Packages require payment in full at the time of purchase. Packages are non-transferrable and non-refundable. Remaining visits will be credited toward future treatment.
A credit card will be kept on-file for any unpaid balances.

Monthly Maintenance Plan
Requires auto-debit from a credit card on-file.
Upon agreeing to the “monthly maintenance plan” you are agreeing to have your credit card charged on the first of each month until canceled.
Cancellation of the monthly charge must be done in-person, over the phone, or in writing and may be done at anytime.
Unused visits will “roll-over” and may be used at any time within one year of purchase.
Visits accrued under the Monthly Maintenance Plan are non-transferrable and non-refundable.
